​Refers to services to the appropriate providers without the hassle of you are having to figure out who they are. It also involves such things as placing loved ones into nursing homes and the process of working through end of life care & death associated paperwork.

From finding trusted professionals to guide you through placing loved ones into nursing homes, to managing the paperwork associated with end-of-life care and bereavement, we're here to support you every step of the way.
Session Details:

Duration: 1-hour system navigation sessions with RN or RPN

Cost: $100/hour for private session

Cost: $40 for workshop session (single person)

Cost: $5o per family (more than 1 person, limit of 3 in the same family)
